Job Description

Schedule: Typically 36 hours per 6-week schedule period; No weekends/holidays/on-call

Contingent, non-benefitted

Shift:  7am-5pm (Depending on office hours)

Will be required to travel to different Primary Care locations within the Jackson area.

 

GENERAL SUMMARY: Leads the coordination and delivery of high-quality, compassionate patient care in an ambulatory family medicine setting. Applies the nursing process—assessment, planning, implementation, and evaluation—to provide, delegate, supervise, and document patient care and education. Collaborates effectively with the interdisciplinary healthcare team while demonstrating clinical competence and maintaining current knowledge in specialty areas. Works both independently and collaboratively to support clinical operations and patient outcomes.

KEY RESPONSIBILITIES:

• Assess patient health status and develop individualized nursing care plans using evidence-based clinical knowledge
• Delegate and supervise nursing tasks while maintaining accountability for quality and safety standards
• Document patient interactions, interventions, and outcomes accurately and timely in accordance with regulatory requirements
• Educate patients and families on health conditions, treatment plans, and preventive care measures
• Communicate clearly and professionally with patients, families, and healthcare team members
• Maintain current clinical knowledge and competencies in specialty areas through ongoing professional development
• Perform a wide range of professional nursing services appropriate to the ambulatory care setting
• Support clinical research initiatives and collaborate with principal investigators as assigned
• Adhere to all regulatory requirements and organizational policies governing nursing practice in Michigan

Qualifications

EDUCATION/EXPERIENCE REQUIRED:

• Graduate from an accredited school of nursing
• Bachelor's degree preferred
• Successful completion of HFMG Pharmacology test
• Preferred experience in ambulatory family medicine or related specialty
• Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans
• Ability to perform a wide variety of professional nursing services appropriate to the ambulatory care setting
• Maintains and demonstrates current clinical knowledge in area of specialty

CERTIFICATIONS/LICENSES REQUIRED:

• Current licensure by the Michigan State Board of Nursing as a Registered Nurse
• Current BLS (Basic Life Support) or ACLS (Advanced Cardiac Life Support) per HFHS policy
• Specialty certifications as required by Michigan State regulatory requirements or documented community standards (e.g., Advanced Cardiac Life Support [ACLS], Oncology Certified Nurse [OCN])

SKILLS/COMPETENCIES:

• Ability to effectively communicate with patients, families, and medical group personnel
• Basic computer knowledge and keyboarding skills
• Strong organizational and time management abilities
• Ability to work both independently and collaboratively within an interdisciplinary healthcare team
