Overview
Physical Therapist – Home Health in Bangs, Texas. This travel assignment in a long-term care setting begins 07/06/2026 for 13 weeks. Guaranteed 36 hours per week. Days shift. Weekly pay range: $2,154–$2,298 per week.

Responsibilities
- Perform comprehensive PT evaluations for residents in a LTC setting and develop individualized treatment plans to address functional goals.
- Implement therapy interventions including therapeutic exercises, gait and balance training, transfers, mobility activities, and functional tasks.
- Monitor patient progress; adjust treatment plans and goals as needed; document outcomes and plan for discharge or transition of care.
- Collaborate with nursing and therapy colleagues to coordinate care, communicate progress, and support interdisciplinary care planning.
- Educate residents and families on home exercise programs, safety awareness, fall prevention, and strategies to maintain gains after discharge.
- Ensure safety, proper body mechanics, infection control, and adherence to facility policies and regulatory requirements.

Requirements
- Current Texas Physical Therapy license (active) or eligibility for licensure to practice in Texas.
- Doctor of Physical Therapy (DPT) degree or equivalent from an accredited program.
- Experience in home health or long-term care settings is preferred.
- Ability to work effectively in a fast-paced LTC environment and to travel as required for a temporary assignment.
- Strong documentation, communication, and teamwork skills to support interdisciplinary care.
