Admissions Registered Nurse

Admissions Nurse Hours: Tuesday–Friday, noon to 10 pm Responsibilities Complete an initial assessment of patient and family to determine home‑care needs and identify problems for each patient upon admission. Outline patient/family referral and admission process. Use assessment data to determine care plan needs and initiate the plan of care. Articulate hospice philosophy, goals and objectives. Communicate with other members of the clinical team to coordinate the care plan. Market to acute hospitals in the area.
