Position Details


Role: Family Nurse Practitioner (FNP)


Setting: 100% Outpatient - Family Medicine (Birth+)


Schedule: Full-time, Monday-Friday (no weekends)


Flexibility: Flexible scheduling options available


Call: No call required

What You'll Do

• Provide comprehensive primary care services to patients of all ages
• Conduct routine physical exams and health assessments
• Diagnose and manage acute and chronic conditions
• Develop individualized treatment, education, and care plans
• Collaborate closely with physicians and interdisciplinary care teams
• Coordinate referrals to specialists as needed
• Deliver high-quality, patient-centered care in a supportive environment

Qualifications


Education: APRN - Family Nurse Practitioner


Licensure: Eligible for or active Connecticut APRN license


Experience: Minimum 1 year in a primary care or outpatient setting preferred
