Bilingual Patient Intake Coordinator (Spanish/English)

We are currently seeking a dedicated and Bilingual (Spanish/English) Patient Intake Coordinator to join our team at our Bronx location. As a Patient Intake Coordinator, you will play a crucial role in facilitating the enrollment process for individuals seeking home care services. Responsibilities Explain program benefits, and conduct initial eligibility (Medicaid and Managed Long-Term Care) reviews and screenings for individuals interested in participating for home care services. Conduct outbound calls to an assigned census of 40-50 active patient cases and answer incoming calls with a goal of 30+ monthly conversations converted into qualified leads. Coordinate and manage the intake process, including gathering necessary documentation and information. Collaborate with healthcare professionals, caregivers, and clients to ensure smooth onboarding and enrollment procedures. Maintain accurate and confidential records of all intake activities and participant information. Provide exceptional customer service by educating clients and caregivers on program guidelines and procedures and addressing inquiries, and concerns, throughout the intake process. Requirements Fluency in both English and Spanish (written and verbal) is essential. Experience in a similar role in Intake or Onboarding home care clients. Strong organizational skills with attention to detail. Excellent communication and interpersonal skills. Ability to work independently and collaboratively in a team-oriented environment. Proficiency in basic computer applications, (Salesforce a plus) and data entry. Benefits Competitive yearly salary within the range of $37,500 - $44,000, based on experience and qualifications.
