Case Manager RN

Role: The Case Manager RN performs inpatient clinical reviews and coordinates care transitions for members in various healthcare facilities. Responsibilities: Conducts onsite or telephonic reviews, collaborates with interdisciplinary teams, manages caseloads efficiently, documents accurately, and ensures appropriate care levels. Participates in patient care conferences, assesses discharge needs, and communicates with facility staff and families to develop discharge plans. Maintains current licensure, adheres to policies, and consults with medical directors on complex cases. Qualifications & Skills: Requires a Bachelor's or Associate's degree in Nursing, an active RN license, 4+ years of clinical experience, and 3+ years in managed care or case management. Knowledge of utilization management, discharge planning, and proficiency in Microsoft Office is essential. Preferred qualifications include experience with psychiatric and geriatric populations and bilingual skills. Physical & Mental Requirements: Ability to lift up to 25 pounds, sit and stand for extended periods, operate office equipment, and use problem-solving skills.
