Key Responsibilities\n

• Conduct thorough ongoing assessments to accurately identify patient needs, ensuring a tailored approach to each individual's mental health journey.

• Facilitate open communication among supervisors, direct care staff, patients, and their families to keep all parties informed of the patient’s progress and necessary adjustments to treatment.

• Develop and maintain an organized schedule for appointments, home visits, and essential observations to track patient advancements seamlessly.

• Coordinate home visits and necessary evaluations, ensuring comprehensive data collection through chart reviews and staff interviews.

• Implement and uphold effective program plans while guaranteeing adherence to ethical and legal standards specific to counseling practices.

• Actively participate in Interdisciplinary Team Meetings, collaborating with other health professionals to discuss case progress and strategies.

• Provide comprehensive monthly and quarterly progress notes reflecting updates on patient treatments and outcomes.

• Organize and lead training sessions for staff to ensure efficient implementation of program protocols and documentation practices.
\n
• Conduct follow-up interviews and home observations to continue support and adjust care plans as needed.

• Maintain detailed records to ensure relevant information is readily accessible for all team members and stakeholders.

• Establish and cultivate relationships within the broader mental health community, advocating for the needs and development goals of patients.

• Assist in crafting procedures and training materials that enhance program efficacy and patient engagement.

Patient Population / Client Focus\n

As a licensed professional counselor, you will work with a diverse patient population including both adults and youth, addressing a variety of mental health concerns. Compensation and Benefits\n

We offer a competitive salary ranging from USD 90,000 to 100,000 per year, commensurate with experience and caseload.
