Key Responsibilities\n

You will play a vital role in the community as you:

• Provide individualized therapeutic support to clients experiencing major mental illness and severe emotional disorders, both virtually and through community engagement.

• Collaborate with a multi-disciplinary team to develop integrated care plans that address the holistic needs of clients.

• Conduct evidence-based screenings, determine appropriate interventions, and participate in ongoing assessments of client progress.

• Participate actively in crisis intervention as needed, ensuring that clients are safe and supported in their moments of need.

• Maintain accurate documentation of all client interactions and ensure timely submission of reports to meet organizational requirements.

• Monitor clients' treatment plans, assist with admissions, ongoing support, and discharge processes.

• Provide timely on-call support during and after program hours as assigned.
