Home Health Occupational Therapist

We are seeking a compassionate Occupational Therapist (OT) to deliver home health services by evaluating patients, creating individualized treatment plans , and providing therapeutic interventions to enhance independence and quality of life. Key Responsibilities Include Assessing patients and developing personalized treatment strategies Administering occupational therapy treatments per the Plan of Care Monitoring patient progress and adjusting plans accordingly Documenting evaluations, treatments, and outcomes accurately Educating patients and caregivers on adaptive equipment, home safety, and exercises Collaborating with physicians and interdisciplinary team members Ensuring compliance with all relevant regulations and standards Qualifications And Skills Required Include Active Florida OT license Strong clinical assessment, documentation, and communication skills Ability to work independently with a flexible schedule Reliable transportation and valid driver’s license Preferred skills involve experience with EMR systems , knowledge of Medicare guidelines , and bilingual abilities (English/Spanish).
