Responsibilities
- Conduct initial and ongoing physical therapy evaluations to determine treatment needs and goals.
- Develop and implement individualized plans of care to restore mobility, strength, balance, and function.
- Deliver evidence-based interventions, including therapeutic exercises, gait training, mobility activities, and ROM work.
- Monitor patient progress, document outcomes, and adjust plans as needed; communicate changes with the care team.
- Collaborate with physicians, nurses, and other therapists to coordinate care and optimize discharge planning.
- Ensure patient safety and adhere to infection control, documentation, and regulatory requirements.
- Provide patient and caregiver education on exercises, equipment use, and home exercise programs.

Requirements
- Active New York physical therapy license (required)
- BLS certification (required)
- Prior experience working in a Skilled Nursing Facility
- Availability to work Saturdays as part of the assignment requirements
- Ability to work effectively in an inpatient/hospital-adjacent setting and follow established PT protocols
