Field-Based Case Manager - Mental Health & Housing (Queens)

Comunilife Inc in Queens is seeking experienced Case Managers for the OMH Scatter Program. You will manage a caseload of approximately 22 clients, complete comprehensive assessments and service plans, and document progress notes. Home visits and fieldwork are required. You will initiate referrals, provide psychoeducation, and collaborate with interdisciplinary teams to ensure client-centered services are delivered and documented.
