We strive to break the cycle of homelessness with personalized, wraparound support, empowering individuals and families to rebuild their lives with stability, dignity, and hope.The Brook permanently houses 189 residents; the majority are formerly street homeless and have serious mental health and medical conditions (including HIV/AIDS) and substance misuse challenges. Our program mission is to help formerly chronically homeless individuals transition into permanent housing with supportive services. BronxWorks at The Brook provides a high level of support to its employees through regular supervision, rounds, on-site trainings, and staff meetings.QualificationsMaster's degree in social work, mental health counseling, or related field requiredMinimum of three years' program experienceProficiency in modern business communications including personal computers, electronic mail, voicemail, and copier equipment.ResponsibilitiesProvide direct services to clientsMaintain case records and statistics for programAssist the program director in recruiting and training direct service staffAdminister personnel policies and procedures under the guidance of the program directorHold monthly supervision meetings with staff supervisedDevelop regular communication with contract managers for programs supervised, act as liaison with funding sourcesEnsure all programs meet contractual goals and reporting requirementsMonitor program progress and submit all required reports to funding sources, including government and private sourcesSubmit internal progress reports monthly, or more frequently as requiredPrepare proposals for the development, expansion and continuation of programs in coordination with Department Director and the Program DirectorMonitor staff performance, including performing case reviews and structured observationsParticipate in, and encourage staff to become involved in, agency eventsAct as liaison with the community including the area community districtPerform additional duties as assignedProgram Specific ResponsibilitiesProvide Clinical Supervisor to Case Managers and Social WorkersServe as a the clinical liaison for onsite medical and psychiatric providersOversee HASA Case ConferencesProvide administrative support in regards to maintaining program complianceAssist with training and recruitment of staffReview all Housing Applications and assess for client service needs prior to movingMaintain case records and statistics for program with the goal of analyzing trends and improving service deliveryAdminister personnel policies and procedures under the guidance of the program directorEnsure all programs meet contractual goals and reporting requirementsSubmit internal progress reports on a monthly basis, or more frequently as requiredMonitor staff performance, including performing case reviews and structured observationsParticipate in, and encourage staff to become involved in agency eventsAct as liaison with the community, including the area community district and boardAct as liaison with medical clinic staff on and off-siteCoordinate clinical services for tenants including discharge planning and hospitalizations in collaboration with team leadersProvide clinical leadership and collaborate with the Program Director to ensure program compliance with DOHMH and HRA contractsProvide direct client care including psychotherapy, diagnosis, and assessment-based treatment planning with tenants who have more intense psychiatric and behavioral health needsCollaborate with tenants to develop an individualized service plan within 30 days of admission into the programProvide conflict resolution, crisis intervention and crisis de-escalation to all tenantsAssess and assist tenants in navigating and accessing services and resources to gain financial self-sufficiencyAssess and assist clients in accessing community based activities of interestAssist in monitoring and our medication monitoring programFacilitate groups and workshops with an emphasis on behavioral health needsActively engage clients who is absent or stops participating in services by conducting appropriate outreach based on the resident's needs, to maintain continuity of servicesEnsure that services are culturally and linguistically competent and delivered in a sensitive mannerDemonstrate in contact notes that services and interventions are related to the goals and objectives of the service plan reviews, and indicate the resident's progress toward the stated goals and objectivesSkillsStrong writing and oral communication skillsStrong presentation skills and ability to represent the agency at functions and meetingsAbility to collect and analyze client demographics and outcomeExcellent organizational, teambuilding and leadership skillsAbility to use a computer for prolonged periodsAbility to occasionally
