Immediate need for a talented Clinical Pharmacist. This is a Full-Time opportunity with long-term potential and is located in Ft. Walton Bch, FL (Onsite).

Job Diva: 26-25061 Pay Range: $52/hr - $64/hour. Key Responsibilities:

• Shift: Days | Rotating Weekends
• Review, interpret, and accurately verify medication orders and prescriptions in the pharmacy system.
• Safely compound and dispense medications while ensuring accuracy and compliance with pharmacy standards.
• Monitor patient medication therapy for drug interactions, allergies, contraindications, dosing appropriateness, and therapeutic effectiveness.
• Provide clinical pharmacy services including renal dosing, pharmacokinetics, anticoagulation monitoring, pain management, nutrition support, and IV-to-PO conversion.
• Provide timely medication information and clinical recommendations to physicians, nurses, patients, and other healthcare professionals.
• Support formulary management and promote appropriate, cost-effective medication utilization.
• Identify, document, and report medication errors and adverse drug reactions.
• Maintain accurate documentation of clinical interventions and patient care activities.
• Collaborate with the healthcare team to optimize medication therapy and improve patient outcomes.
• Assist in supervising daily pharmacy operations and provide support to the pharmacy team.

Key Requirements and Technology Experience:

• Experience: 1+ year hospital experience preferred | PGY1 candidates considered
• License: Active Florida Pharmacist License required
• Education: BS or PharmD from an ACPE-accredited pharmacy school.
