We are offering COMPETITIVE PAY with AMAZING BENEFITS and great WORK LIFE BALANCE. We are looking for a dedicated Nurse Practitioner for our Skilled Nursing Facility (SNF) in East Boston, MA. This is a full-time, in-person position, Monday through Friday, 9 to 5, with no nights, no weekends, and no on-call. As the primary care and rounding provider in the facility, you will manage a panel of long-term care and geriatric patients, working independently from facility staff while collaborating on patient care. Qualifications of the Nurse Practitioner: Active Massachusetts NP license in good standing Experience in skilled nursing, long-term care, or geriatric primary care preferred Comfortable rounding independently and managing a full patient panel Strong assessment, documentation, and EMR skills Responsibilities of the Nurse Practitioner: Serve as the primary care and rounding provider for patients in the facility Conduct assessments, manage chronic conditions, and adjust care plans Document visits accurately and maintain compliance with facility and regulatory standards Participate in patient care meetings and coordinate with the interdisciplinary team #J-18808-Ljbffr Gilder Search Group
