As one of the nation's largest nonprofit Catholic healthcare organizations, CommonSpirit Health delivers more than 20 million patient encounters annually through more than 2,300 clinics, care sites and 158 hospital-based locations, in addition to its home-based services and virtual care offerings.As our RN Care Manager, you will coordinate and facilitate comprehensive care plans for a diverse patient population, ensuring seamless transitions and optimal health outcomes.Every day you will assess patient needs, develop individualized care plans, and collaborate with patients, families, and multidisciplinary healthcare teams to integrate medical, social, and psychological aspects of care. You will serve as a central point of contact, advocating for patients, facilitating access to resources, and proactively managing their continuum of care to prevent complications and reduce hospital readmissions.To be successful in this role, you will demonstrate exceptional clinical knowledge, superior communication and advocacy skills, and a strategic, patient-centered approach to healthcare delivery that empowers individuals to achieve their highest level of wellness.Job RequirementsCurrent Washington State licensure as an RNCPR certification is required every two (2) yearsBachelors of Science of NursingStrong patient assessment skills and the ability to work collaboratively in team settingCurrent working knowledge of electronic medical recordsVirginia Mason Franciscan Health brings together two award winning health systems in Washington state - CHI Franciscan and Virginia Mason.
