Registered Nurse (RN) PRN Case Manager Job Type: PRN | Days Schedule: Days, Variable Hours Case Manager RN at Havasu Regional Medical Center, a 171-bed community hospital, offers a hybrid position supporting patients and their families in both inpatient and emergency department settings. Responsibilities Monitors patient and family satisfaction; responds to questions and complaints from patients, families, and payors. Leads and coordinates discharge planning, including patient transfers to other facilities and referrals to community resources; provides discharge education. Performs chart reviews to identify and address issues related to service delivery, patient outcomes, and compliance. Communicates effectively with providers, payors, and ancillary services to remove barriers and ensure timely care progression. Accurately documents case management activities, plans, and outcomes. Qualifications Applicants must have a current Arizona or Compact State RN license and an Associate's Degree in Nursing (ADN) from an accredited nursing school. Additional preferred qualifications include: Bachelor's or Master's Degree. Basic Life Support (BLS) within 30 days of hire; Accredited Case Manager (ACM) within 2 years of hire. 2 years of RN experience; 2 years of Case Management experience preferred.
