Key Responsibilities\n

• Facilitate individual and group therapy sessions that provide emotional support and foster coping mechanisms among patients and their loved ones.

• Be responsive to crisis situations, providing immediate interventions and connecting patients to necessary resources, if required, while adhering to legal and ethical standards.

• Educate patients and families on mental health issues, treatment options, and available support resources, helping them to navigate the challenges of hospice care.

• Maintain meticulous documentation of therapy sessions, treatment plans, and progress in alignment with regulatory requirements.

• Actively participate in team meetings and case conferences, sharing insights and advocating for patients' needs as part of a cohesive care team.

Patient Population / Client Focus\n

You will primarily work with adult patients receiving hospice care, many of whom are experiencing complex emotional responses to terminal diagnoses. Qualifications\n

• Master's degree in Social Work from an accredited institution.

Compensation and Benefits\n
