Erie Family Health Promotions seeks a Health Educator in Waukegan to deliver 1:1 health education visits, facilitate group sessions, and complete disease management assessments. The role also supports the Lending Hands for Life program with HIV-focused group classes and outreach to ensure patients remain in care.
