Health Educator: HIV Care & Community Education
AmeriCorps & Senior Corps
Erie Family Health Promo seeks a Health Educator in Humboldt Park to deliver one-on-one health education visits, facilitate group sessions, and complete disease management assessments. The role also supports our Lending Hands for Life program by conducting group classes for patients with HIV and assisting outreach to ensure care follow-up.
