Hospice Referral Specialist & Community Liaison

Lorian Health is seeking a Hospice Care Specialist/Community Liaison to grow and manage referral relationships with physicians, hospitals, SNFs, and social workers. You will serve as the face of Lorian Hospice, educate partners, and drive awareness and admissions through strategic outreach. This role requires independent territory management, strong communication, and a proven sales track record.
