Neighborhood Healthcare PACE in Riverside, CA is seeking a Referrals Specialist to coordinate referrals and follow-up care for PACE participants. This on-site role handles scheduling, authorizations, and transportation arrangements to ensure timely patient care. Schedule: Monday–Friday, 8:00 am–4:30 pm.
