Work Environment\n

This full-time role offers a remote work setting primarily focused within a Hospice Center. Our organization emphasizes flexibility to accommodate the varying needs of our clients while ensuring continuity of care. You will be required to work Monday through Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours as client needs arise. As a vital member of our team, on-call availability may also be necessary to address urgent situations.

Key Responsibilities\n

• Conduct initial and ongoing assessments of seniors' psychosocial needs, drawing from emotional, mental, and social variables to craft personalized care strategies.

• Collaborate closely with clients and their families to design individualized care plans that reflect their specific preferences, aspirations, and circumstances.

• Provide empathetic counseling and emotional support for seniors and their families, assisting them through transitions related to aging or health challenges.

• Act as an advocate for clients' rights and facilitate access to pertinent community resources to ensure they receive necessary services.

• Work hand-in-hand with interdisciplinary teams, which include healthcare staff such as nurses and caregivers, to improve care quality and coordination.

• Perform regular home visits to oversee clients' living conditions and overall well-being, addressing any potential concerns or risks promptly.

• Facilitate communication among family members to resolve conflicts related to the care of senior clients.

• Educate families about available community support resources and social programs that enhance social interactions and overall life satisfaction.

• Maintain thorough, accurate records of client assessments, service plans, progress notes, and necessary correspondence.

Qualifications\n

Compensation and Benefits\n

Competitive salary ranging from $60,000 to $100,000 annually, commensurate with experience.
