• Conduct comprehensive psychosocial assessments to determine the specific emotional, mental, and social needs of elderly clients.

• Develop tailored care plans in collaboration with clients and their families, reflecting their individual goals and preferences.

• Provide counseling and emotional support to clients and their families, helping them navigate transitions such as health changes or loss of independence.

• Advocate fiercely for seniors’ rights and access to essential community resources, ensuring they receive the necessary services and support.

• Work as part of an interdisciplinary team, coordinating with healthcare professionals to deliver an integrated approach to client care.

• Perform regular client visits to monitor their well-being, assess living conditions, and address any safety or health concerns.

• Educate clients and their families about available community programs, resources, and support groups, enhancing social engagement.

• Facilitate family communication and conflict resolution, ensuring all parties are informed and involved in the care process.

• Maintain thorough and accurate documentation of client interactions, assessments, and progress notes, ensuring compliance with ethical and regulatory standards.

The primary focus of this role is on seniors aged 65 and older, many of whom face unique challenges such as chronic health issues, isolation, and mental health concerns. You will engage with clients from diverse backgrounds, tailoring approaches to meet individual needs and preferences. Your work will not only enhance their quality of life but also strengthen family dynamics and community integration.

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of professional experience in social work, specifically within a community center or geriatric care setting.

• In-depth knowledge of social work practices, aging-related challenges, and community resources.
\n
• Exceptional assessment, listening, and counseling skills, with a demonstrated ability to build rapport with clients and families.

• Proficient in utilizing electronic health record (EHR) systems and other relevant software applications to enhance service delivery and documentation.
