If you’re passionate about making a difference and want a career that grows with you, **Emblem** is the place to be! *\\*This is a **full-time** position offering a **Monday-Friday** schedule! **\\*This position requires travel** and will provide service throughout the **North Phoenix** and **Scottsdale** areas!* **Make a meaningful impact every day.** As a **Hospice RN**, you’ll act as a trusted Case Manager, delivering care that supports patients in their homes and communities. You will guide families through care decisions, while providing comfort, dignity, and compassionate support during life’s most tender moments. You will:* Complete comprehensive assessments of patients and families to determine hospice needs* Develop and implement individualized care plans, revising as needed* Provide skilled nursing care, administer medications, and manage symptoms* Educate patients and families on care techniques, medications, and comfort measures* Maintain accurate clinical documentation and communicate changes to physicians* Participate in interdisciplinary team meetings and quality improvement initiatives* Provide emotional support and counseling to patients and families* Supervise hospice aides and delegate responsibilities appropriately ***Our Commitment to You**** Flexible schedules to fit your life and support balance* Ongoing professional development and training* A truly collaborative team that celebrates your success ***Requirements**** Currently licensed as a Registered Nurse (RN) in the State of Arizona* Minimum of 2 years of nursing experience, hospice or home health experience preferred* Homecare Homebase (HCHB) experience preferred, not required* Valid driver’s license and reliable, insured vehicle* Strong clinical skills and a passion for patient-centered care ***Benefits**** Medical / Dental / Vision Insurance* Paid Time Off* 401(k) with Employer Match* Employee Assistance Program* FSA / HSA* Life Insurance* Pet Insurance* Employee Referral ProgramThe employer for this position is stated in the job posting.
