Work Environment\n

This position offers the flexibility of remote work, allowing you to perform your duties from home while serving clients who primarily reside within our community health clinic. While the regular hours are Monday to Friday from 8:00 AM to 4:30 PM, some evening and weekend hours may be necessary based on client needs. This role requires adaptability and a commitment to providing continuous care, which may involve occasional on-call situations to address urgent client needs.

Key Responsibilities\n

• Conduct comprehensive assessments of clients’ psychosocial needs, ensuring to integrate emotional, mental, and social aspects.

• Collaborate with clients and their families to design and implement individualized care plans tailored to their specific goals and preferences.

• Provide counseling and emotional support to seniors and their families, assisting them in navigating significant life changes and challenges.

• Advocate for clients’ rights and facilitate access to necessary community resources, ensuring they receive appropriate and timely support.

Qualifications\n

• A Bachelor's degree in Social Work from an accredited institution is required.
