Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate the mental health status, strengths, and needs of hospice patients and their families, taking into account the unique challenges associated with end-of-life care.

• Deliver evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), to help patients manage their emotional distress and navigate complex feelings during this critical period.

• Collaborate closely with a multidisciplinary team, including psychiatrists, nurses, and chaplains, to develop and implement coordinated care plans that reflect the patients' values and goals.

• Support patients and their families in understanding diagnoses, medication management, and available treatment options, providing clear and empathetic guidance throughout their journey.

• Facilitate individual, group, and family therapy sessions, ensuring the utilization of effective evidence-based modalities tailored to the hospice context.

• Develop and maintain personalized treatment plans in collaboration with other care team members, integrating patient goals and incorporating feedback from family members.

• Assist patients and their families in navigating the complexities of the mental health system, advocating for their rights and access to necessary resources.

• Organize and lead therapeutic groups and psychoeducation sessions, focusing on coping strategies, grief processing, and preparing for transitions.
