Key Responsibilities\n

• Conduct thorough assessments, establish treatment plans, and provide individual, group, and family therapy tailored to the needs of clients in a hospice setting.

• Develop and implement integrated care plans while ensuring continuity of care through effective communication with clients, family members, and multidisciplinary teams.

• Utilize evidence-based practices to conduct screenings and assessments, addressing immediate needs and arranging for additional support when necessary.

• Facilitate crisis intervention as needed, providing timely and appropriate responses to psychosocial emergencies.

• Regularly engage in team meetings and collaboration sessions to share insights and strategies that enhance overall service delivery and patient outcomes.

• Maintain accurate documentation of clinical interactions and ensure timely completion of reports, data submissions, and other administrative tasks.

Compensation and Benefits\n

The annual salary for this position ranges from USD 70,000 to 85,000, based on qualifications and experience.
