Public Health Representative 2The Vulnerable Populations Coordinator (VPC) plays a critical role in supporting the Healthcare Coalition (HCC) and Regional Health Department to enhance preparedness, response, and recovery efforts for at-risk populations. These populations may be disproportionately affected during disasters and public health emergencies.Working under the guidance of the Tennessee Department of Health (TDH) and/or the Regional Healthcare Coordinator (RHC) / Emergency Response Coordinator (ERC), the VPC leads outreach, planning, training, and response initiatives aimed at reducing health disparities and advancing health equity across the region.Key Responsibilities include:Collaborate with Healthcare Coalitions (HCCs), healthcare facilities, and community organizations to integrate vulnerable population needs into emergency preparedness and response planningBuild and maintain partnerships with long-term care, hospice, home health, dialysis, assisted living, and behavioral health providersSupport vaccination efforts and infectious disease response, including planning and operating Points of Distribution (PODs)Provide training and technical support on Healthcare Resource Tracking System (HRTS) and Patient Bed Matching (PBM); act as regional subject matter expert (SME) for PBMMaintain coalition contact lists, inventories, and preventive maintenance logsDevelop and update resource directories for agencies supporting vulnerable populations (e.g., medical suppliers, food banks, housing services, NGOs)Lead outreach efforts and coordinate quarterly meetings with partner organizationsCollaborate with government agencies, NGOs, and local coalitions to strengthen community engagementSupport regional exercises and contribute to After-Action Reports and Improvement PlansAssist with reporting, data management, and compliance with ASPR HPP and CDC PHEP requirementsProvide logistical and operational support during emergency activationsPerform additional duties as assignedKnowledge, Skills & Abilities include:Strong understanding of public health, emergency preparedness, and healthcare systemsKnowledge of social vulnerability and access/functional needs in disaster planningAbility to design and deliver training programsExcellent communication, facilitation, and stakeholder engagement skillsProficiency in Microsoft Office Suite and preparedness systems (HRTS, PBM)Strong data management and organizational skillsQualifications include:Bachelor’s degree in Public Health, Emergency Management, Healthcare Administration, IT, or related field (Master’s preferred)2–3 years of relevant experience in public health, emergency management, or community outreachRequired certifications: ICS 100, 200, 700, 800Preferred: ICS 300/400 (within 24 months of hire)Valid Tennessee driver’s license with ability to travel regionally and statewideWork Environment includes:Hybrid role with office, community, and statewide travelRequires occasional lifting (up to 30 lbs) and a mix of sitting and standingAvailability for after-hours emergency response and extended shifts during activationsModerate exposure risk during public health emergency operationsWork Schedule:Days: Monday – FridayHours: 7.
