Remote RN Case Manager (Costco Team) Location: Remote (Preferred: San Antonio, TX area) Travel to the San Antonio office may be required if you live within 45 minutes of their location. Schedule: Monday–Friday, 8:00 AM–5:00 PM CST (occasional 10:00 AM–7:00 PM rotation) No weekends or holidays About the Role Join our dedicated Costco Team as a Remote Case Manager RN, where you’ll play a key role in improving members’ health outcomes. In this role, you’ll assess, plan, and coordinate care—helping members navigate their healthcare needs and achieve better overall wellness. What You’ll Do Conduct comprehensive telephonic (and occasional in-person) assessments Develop and implement personalized care plans Identify and address clinical, social, and behavioral health needs Collaborate with multidisciplinary teams to support holistic care Guide members through benefits, resources, and care transitions Use clinical tools and data to reduce risks and improve outcomes What You’ll Bring - REQUIRED Active, unrestricted multi-state RN license in state of residence 3+ years of clinical RN experience 1+ year experience using multiple systems (e.g., Excel, Outlook, web-based tools) Strong assessment, communication, and critical thinking skills Willingness to obtain additional state licenses (company-paid) Preferred Qualifications Case management experience (integrated model a plus) Bilingual (Spanish/English) BSN preferred Why Join Us? Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $60,522.00 - $129,615.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
