Behavioral Health Discharge Coordinator

Terros Health is seeking a Discharge Planner to serve as the liaison between inpatient settings and the assigned clinician at the Outpatient site. The role emphasizes coordinating transitions, identifying community resources, and monitoring follow-up with psychiatric providers. The position requires a High School Diploma or GED, or a bachelor’s degree in behavioral health with relevant experience.
