Shift & Schedule: Days: 7:30 AM – 4:00 PM | Evenings: 3:30 PM – 12:00 AM | Nights: 11:30 PM – 8:00 AM

Pay Rate

Local: $35 - $40/hr
Traveler: $40 - $43/hr

Position Overview

We are seeking an experienced LPN to provide direct nursing care, medication administration, clinical treatments, and resident monitoring in a long-term care setting.

Required Qualifications

• Active LPN License.

• Current BLS certification.

• Long-Term Care (LTC) or Skilled Nursing experience preferred.

• Strong medication administration and documentation skills.

• Ability to supervise CNAs and collaborate with the healthcare team.

Responsibilities

• Provide direct resident care, including bathing, dressing, grooming, toileting, feeding, and mobility assistance.

• Administer oral, topical, injectable, and inhaled medications as prescribed.

• Monitor vital signs, weights, oxygen therapy, and changes in residents’ physical or emotional condition.

• Perform wound care, dressing changes, skin assessments, and preventive skin care.

• Collect laboratory specimens and report abnormal findings to the RN or provider.

• Insert and monitor urinary catheters, feeding tubes, and IV therapy as permitted by state regulations.

• Supervise and guide CNAs and ensure care plans are followed.

• Accurately document nursing care, assessments, and resident status changes in the EMR.

• Participate in shift handoff, care planning, and interdisciplinary care conferences.

• Respond to emergencies, provide CPR/first aid, and promptly notify the supervising RN of changes in condition.
