Physical Therapist

Physical Therapist Job Summary: Plans, organizes, and implements Physical Therapy services for PACE participants. Responsibilities include, but are not limited to, assessment; planning, providing treatment; and teaching participants, caregivers, or other appropriate representatives/family to maintain participant support in the community. Provides high quality care as a member of the PACE interdisciplinary team (IDT). Performs in‑person assessments for participants at initial enrollment and as needed throughout care. Assesses need for adaptive durable medical equipment, specialized shoes, and prosthesis. Assesses participants’ homes for safety and mobility; recommends modifications and equipment. Plans physical therapy treatment programs according to recognized standards and policies. Administers treatment programs to achieve identified goals and according to standards and policies. Collaborates with the interdisciplinary team on participant care issues. Documents evaluation, assessment, plan, and treatment rendered accurately and timely. Measures effectiveness of treatment, documents and revises plan accordingly. Educates and instructs participants/family in procedures, home programs, and ensures understanding. Trains PACE staff in body mechanics, equipment use, transfers, gait patterns, and safety methods for ADLs. Provides care and services in the center, clinic, contracted facilities, and participants’ homes per plan of care. Assists participants to function at the most independent level possible. Develops and maintains positive relationships with co‑workers, participants, families, and community members. Assists interdisciplinary team members in understanding significant rehabilitation needs. Collaborates with the interdisciplinary team to develop comprehensive care plans. Participates in daily Interdisciplinary Team meetings, communicates changes, and collaborates on care planning. Performs duties of other IDT members as needed based on licensing, competencies, and experience. Participates in quality improvement projects and meetings. Attends organizational meetings related to participant care, daily operations, in‑services, and community agency meetings. Completes all clinical documentation in participants’ medical records per PACE requirements. Follows all Policies and Procedures and OSHA safety guidelines. Protects privacy and maintains confidentiality of all employee, participant, and family information. Practices standard precautions and maintains a safe working environment. Participates in continuing education classes and required staff training meetings. Maintains professional affiliations, required certifications, and continuing education commitments. Performs other duties as assigned. Experience, Education, and Certifications Bilingual and fluent in Hmong preferred. Advanced degree in Physical Therapy from an accredited school. Current license to practice Physical Therapy in the state. Current driver’s license and proof of auto insurance. Two (2) years of experience on an interdisciplinary team in a hospital, nursing home, or community‑based setting preferred. One (1) year of experience working with a frail or elderly population preferred. Training will be provided upon hiring if required experience is not present.
