This full-time remote position focuses on delivering compassionate support to seniors in hospice care. Work Environment\n

This role allows for flexibility, primarily working remotely while collaborating with team members in a hospice center. Our workweek runs Monday through Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours required based on client needs. With an emphasis on responsive care, you may also be called upon to provide support outside regular hours to address urgent situations that arise.

Key Responsibilities\n

As a Licensed Master Social Worker with Self Harmony Counseling, you will:

• Conduct detailed assessments of the psychosocial needs of elderly clients, analyzing emotional, mental, and social factors affecting their well-being.

• Collaborate extensively with clients and their families to co-create personalized care plans that align with specific goals and personal preferences.

• Provide empathetic counseling and emotional support to seniors and their families, facilitating discussions that help them navigate significant life transitions and challenges.

• Advocate for clients' rights, ensuring they have access to vital community resources, services, and support systems.
\n
• Coordinate care with a diverse team of healthcare professionals—nurses, caregivers, and other specialists—to optimize client outcomes.

• Conduct regular home visits to monitor client well-being, evaluate their living conditions, and address any emerging safety concerns.

• Serve as a mediator to facilitate effective communication and resolve conflicts among family members involved in client care.

• Inform clients and their families about available community resources, support groups, and programs designed to enhance social engagement and improve quality of life.

• Maintain meticulous records of client assessments, care plans, progress notes, and all communications, ensuring compliance with current social work standards.

• Stay informed on best practices in social work and participate in ongoing professional development to enhance skills and knowledge.

Patient Population / Client Focus\n

In this role, the focus will be primarily on the elderly, especially those facing life-limiting illnesses. You will work with clients within the hospice care setting, providing emotional and psychosocial support to enhance their quality of life in their most vulnerable moments. Building strong relationships with both clients and their families will be essential to delivering holistic care that nurtures body, mind, and spirit.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of relevant experience in a social work role, preferably in geriatric or hospice care settings.

Compensation and Benefits\n

The position offers a competitive salary range of $60,000 to $100,000 annually, commensurate with experience.
