Hospice Physician

St. Peter's Health Partners is looking for a hospice physician in a hospice home care setting. The reason for coverage is retirement and home care. Dates of coverage are ASAP until indefinitely. Call schedule is on average one weeknight per week from 4pm to 8am and one in four/five weekends from 4pm on Friday until 8am on Monday. Call ratio is 1 in 4/5. The average number of patients per day is variable. The majority of the hospice physician's time is spent in interdisciplinary team meetings, reviewing cases for hospice eligibility, and providing guidance and recommendations to the nurses in the field. Home visits for face to face encounters and symptom management are scheduled as needed in accordance with the hospice physician's schedule on average 0-2 per day. Documentation system/EMR is EPIC - Comfort/Hospice Application. Provider must be local - only mileage is reimbursed. Trinity Health does not reimburse for credentialing costs or application fees.

Submission requirements: Board certified - internal medicine / family medicine / emergency medicine - required. Hospice and palliative medicine or geriatrics fellowship training is a plus but not required. NY state license - required. BLS - required. Provider must be local - required. Conversion rate cap of ***k - required.
