Overview A Home Health Quality Nurse Coordinator positions at a top‑ranked medium‑sized hospital in Southern Georgia. The role reports to a Director of Quality and works in a collaborative dotted line with the Home Health Administrator and Infection Prevention leadership. Responsibilities Lead PDSA and Lean Six Sigma projects. Ensure OASIS accuracy and HHVBP performance. Drive survey readiness and compliance with CMS Conditions of Participation and accrediting bodies. Advance antimicrobial stewardship and home‑based infection prevention practices. Optimize clinical decision support and order sets. Mentor staff through education and competency validation.
