BrightStar of Lake County is seeking an experienced and dependable Registered Nurse (RN) for a full -time hybrid role combining skilled field nursing with office-based clinical support. This role is ideal for an RN who is confident with: -Skilled nursing assessments and evaluations -Infusion therapy -Wound care management -Care planning and Clinical oversight Unlike typical PRN home care roles, this position offers consistent week day hours, long-term patient relationships, and the opportunity to grow into clinical leadership or case management. RN Responsibilities Skilled Field Nursing -Perform nursing assessments, admissions, reassessments, and supervisory visits -Provide skilled nursing care in patients' homes -Perform wound care and monitor healing progress -Manage and support infusion therapy cases -Identify changes in patient condition and communicate appropriately -Educate patients and families on medications, treatments and ongoing care needs -Develop, implement and update individualized care plans Office / Clinical Support Duties: -Review clinical documentation for accuracy and compliance -Process and track physician orders -Conduct caregiver skills assessments and competency evaluations -Assist with caregiver on-boarding and orientation -Collaborate with scheduling and leadership to ensure continuity of care -Communicate proactively with the Director of Nursing regarding patient and staff needs What We’re Looking We are seeking a clinically strong RN who is: -Compassionate and patient-focused -Organized and detail oriented -Comfortable working independently -Skilled in documentation and clinical communication -Dependable, accountable, and professional -Interested in long-term growth within a supportive clinical team Requirements -Active, unrestricted Indiana RN license -CPR certification -Valid driver's license and auto insurance -Negative TB test or chest x-ray -Ability to lift up to 50 lbs. and assist with patient mobility -Home care, infusion, wound care or supervisory experience preferred Pay & Benefits -Up to $45/hour based on experience -Monday - Friday schedule -Occasional weekends or on-call support as needed -Weekly pay with direct deposit -401 (k) with employer match -Paid time off -Health, dental, and vision insurance available for employees working 20+ hours/week Additional Benefits -Vitable Health (for eligible team members): -$0 copay primary care (in-person and virtual) -$0 copay mental health therapy and life coaching -Nearly 1,000 prescriptions at $0 cost -Coverage for entire household ZayZoon Pay on Demand: -Access earned wages instantly -No waiting for standard payroll About BrightStar Care of Lake County BrightStar Care is a nurse-led home care agency focused on clinical excellence and employee support.
