Continuum Therapy Partners is seeking a compassionate and dedicated Physical Therapist (PT) for a consistent PRN opportunity at Evergreen Nursing and Rehab in Effingham, Illinois. This position offers a predictable schedule of approximately 2-3 days per week (8-12 hours per week) with the flexibility to consider part-time or full-time opportunities as well. As a Physical Therapist (PT) , you will provide high-quality therapy services to help patients improve mobility, strength, balance, and functional independence. Responsibilities Evaluate patients and develop individualized physical therapy treatment plans. Provide skilled physical therapy interventions to improve mobility, strength, balance, and independence. Perform therapeutic exercises, gait training, mobility training, and functional activities. Monitor patient progress and adjust treatment plans based on patient needs. Collaborate with physicians, nursing staff, caregivers, and interdisciplinary rehabilitation team members. Complete accurate and timely documentation according to company, state, and federal guidelines. Educate patients and families on mobility strategies, exercise programs, and safety techniques. Provide compassionate, patient-centered physical therapy services. Qualifications Graduate of an accredited Physical Therapy program. Current Illinois Physical Therapist (PT) license required. Skilled nursing or rehabilitation experience preferred but not required. New graduates are encouraged to apply. Strong communication, organization, and teamwork skills. Passion for providing high-quality patient care.
