Work Environment

This position is located within a school setting, providing a unique opportunity to work closely with children and families. Our facility embraces a collaborative approach, encouraging communication among healthcare providers to deliver unified care. Typical work hours span from 9:00 a.m. to 6:00 p.m., but may extend to 8:00 p.m. for evening clinics when needed. Participation in weekend outreach events or health fairs will also be part of your schedule, further solidifying our commitment to community engagement. Expect to work an average of 40 hours per week, enabling a good work-life balance.

Key Responsibilities

• Conduct comprehensive health assessments for individuals and families, ensuring a thorough understanding of medical histories and current health issues.
• Diagnose and manage both acute and chronic health conditions, developing tailored care plans that promote preventive care and health education.
• Collaborate with schools and community organizations to design and implement health education programs aimed at increasing awareness about preventative health measures.
• Advocate for patients’ rights, helping to navigate the healthcare system, and providing culturally sensitive care tailored to the unique needs of diverse populations.
• Coordinate referrals to specialists and ensure appropriate follow-up care, enhancing continuity and quality of care for our patients.
• Maintain accurate and detailed medical records in compliance with healthcare regulations, facilitating effective communication within the care team.
• Engage in community outreach initiatives to provide health screenings and enhance awareness regarding health resources available in the area.
• Stay updated with the latest developments in community health trends, evidence-based practices, and pertinent healthcare policies.

Patient Population / Client Focus

Your role will primarily focus on families, including children, and individuals from varied demographics within the local community. You will specifically address the needs of underserved populations, ensuring equal access to healthcare services and promoting health equity. Qualifications

• Current certification as a Family Nurse Practitioner.
• Active state nursing license in New York.
• A Master's degree in Nursing from an accredited program.
• A minimum of three years of clinical experience, preferably in community health settings.
• Exceptional interpersonal and communication skills, enabling effective collaboration with diverse populations and professional teams.

Preferred Experience

Compensation and Benefits
