Key Responsibilities

• Conduct comprehensive evaluations of patients by reviewing pertinent medical history, performing targeted assessments, and developing individualized treatment plans based on the evaluation findings.
• Communicate evaluation results and treatment strategies effectively to both patients and their families, fostering collaboration in goal-setting to enhance rehabilitation outcomes.
• Implement and adapt treatment protocols in accordance with the patient's progress and evolving healthcare practices, ensuring the effectiveness of interventions.
• Collaborate with other healthcare professionals to optimize patient care and address any changes in patient condition, modifying the plan of care as necessary.
• Complete thorough discharge planning and follow-up care documentation, ensuring accurate and timely records of patient encounters that reflect their comprehensive treatment journey.
• Manage your schedule and meet departmental productivity expectations while maintaining punctuality for appointments and meetings.
• Engage actively in departmental goal setting and compliance with relevant regulations, contributing to the overall success of the facility.

Patient Population / Client Focus

The primary patient population includes individuals recovering from orthopedic surgeries, sports-related injuries, neurological conditions, and post-operative rehabilitation. Our focus is to provide patient-centered care, emphasizing active participation in their healing process, and addressing their specific needs through personalized therapy plans.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• High School Diploma or equivalent.
• Current physical therapy license as per state requirements.
• Certification in Basic Life Support (BLS) for Healthcare Providers by the American Heart Association.
• No previous experience is required; however, passion and commitment to patient care are essential.

Preferred Experience

While not mandatory, experience in outpatient physical therapy settings, familiarity with electronic health record systems, or specialization in orthopedic rehabilitation will be considered advantageous. Candidates who have actively participated in clinical rotations or internships in similar environments may also be viewed favorably.

Compensation and Benefits
