Weekend On-Call Hospice Care Manager RN

Gentiva seeks an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will perform assessments, admissions, and routine visits with primary clinical oversight for assigned patients, working within the IDG to coordinate care with physicians and families. The role emphasizes after-hours nursing leadership, documentation, and regulatory compliance, with weekend shifts from Friday 5pm to Monday 8am.
