Overall SummaryUnder the general direction of the Behavioral Health and Social Services Director, leads the day-to-day operations of the behavioral health care coordinators within the Behavioral Health Department. Provides support to BH care coordinator staff with patient and work Issues; coordinates break and lunch coverage throughout the day and time-off requests. Monitors and ensures the quality of coordinators’ work by engaging in regular performance evaluations and creating performance improvement plans, as needed. Works with BH care coordinators to ensure referrals are managed/updated, reminder contacts are completed, documentation is accurate and timely, and completes warm connection requests from PCP and other staff. Develops and updates BH care coordinator workflow and policy and procedures as needed. Ensures BH care coordinators are participating in regular check-ins to prepare for dally operations. Monitors staff for completion of duties, assists with scheduling, training and oversight as directed by the BH Manager. In support of patient care at Bay Area Community Health (BACH), provides a variety of clinical and support duties and language Interpretation as needed.Essential ResponsibilitiesUnder the general direction of Behavioral Health Manager, Behavioral Health Social Services Supervisor performs a variety of complex, professional, analytical, and confidential behavioral health services.Duties include:Monitors patient flow throughout the day, including waiting times, problem solves and addresses Issues to ensure delivery of quality patient services. Informs BH manager of any urgent Issues or concerns (patient, staffing, and facility Issues).Provides support to BH care coordinator staff with patient Issues, coordinates break and lunch coverage throughout the day.In collaboration with BHSS Director, organizes BH Care Coordinator work schedules, approves PTO requests, and ensures that daily staffing coverage needs are met.Supports BH care coordinators with patient care, connecting to referrals and following up with medication management.Assists with training and monitors the work of other BH care coordinators, maintains documentation and provides performance feedback to BH Manager.Participates in annual performance review of BH care coordinators.In collaboration with BHSS director, review utilization of BH providers on regular basis.Accepts warm connections between agencies and coordinates care to facilitate Integration of care.Accepts warm connections of patients between medical and behavioral health providers to facilitate Integration of care.Assists in establishing, maintaining smooth functioning, and relationships between primary care providers, behavioral health specialists, and primary care consulting psychiatrists.Identifies essential client resources and refers patients to other clinic departments, external agencies, or community resources for appropriate treatment, services, and supports.Tracks client attendance and follows up on no-shows to support and Improve continuity of treatment.Facilitates weekly clinical meetings, and supports team in following up with action steps.Facilitates communication for mental health provider schedules with a focus on optimizing productivity.Provides referrals to community resources, liaison with medical and mental health providers, and troubleshooting integration efforts.Participates in referrals for clients who are experiencing a crisis or higher level of care need to appropriate service providers and follow up with individuals to confirm contact with service provider and if further services are needed.Gathers data and coordinates reports for Primary Care Psychiatric Consultation Program (PCPCP) to assure program compliance.Responsible for data collection, and participates in the generation of IBHP reports.Secondary ResponsibilitiesAttends workshops, training, and meetings as needed, and as requested.Performs other duties as assigned by supervisor.Qualifications:Bachelor’s Degree (Healthcare preferred) or 4-6 years of progressive work experience (typically in Healthcare Programs) with acquired knowledge at the level of a Healthcare Specialist.Strong organizational, administrative and problem-solving skills and ability to be flexible and adaptive to change while maintaining a positive attitude.Ability to prioritize tasks; work under pressure and complete assignment in a timely manner.Ability to effectively present information to others, including other employees, community partners and vendors.Ability to seek direction/approval from on essential matters, yet work independently with little oversight.Supervision of staff using professional judgment, diplomacy, ethics, and knowledge of staff professional development needs and agency responsibilities to governing agencies and grant funders.Work in a team-oriented, multi-disciplinary environment using a flexible and adaptive communication and interpersonal approach.Job Experience Requirements:The equivalent of at least three years of full-time work experience in a health care organization as a community health outreach worker, or patient services technician performing duties, such as Intake, peer counseling, obtaining personal history, data collection/analysis, non-clinical assessments, client health education, case management.Working with mental health and/or substance abuse population.Job Preferences:Knowledge of Bay Area and social services resources.Experience providing services to persons who are disabled, homeless, substance users, re-entry population, and/or psychologically impaired.Proficient in Microsoft office word with ability to manage databases.Bilingual strongly preferred.Work Conditions:Travel locally to visit other sites, may require a day or overnight trip to attend events; workload may be demanding at times; require professionalism and confidentiality.Physical Demands:This position requires:Sitting, walking, and standing associated with a normal medical, clinical and administrative office environment, with some bending, reaching, stretching, and lifting up to 25 pounds.Manual dexterity for tasks involving standard office equipment and computer keyboard, including repetitive movements, such as typing.Traveling to work sites, including some exposure to outside elements.Exposure to potentially dangerous materials and situations, requiring adherence to extensive safety precautions and possibly the use of protective equipment.Supervisor:Managerial personnel (supervisors, managers, directors, executive management) who are exempt or non-exempt and who are responsible for use of independent judgment to make personnel decisions or to recommend personnel decisions.
