RESPONSIBILITIES AND DUTIES: * Provide direct patient care through office visits, telehealth, and occasional same-day appointments * Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions * Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions * Educate and empower patients on preventive health and functional medicine principles * Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff * Document visits in the EHR in a timely and compliant manner * Participate in team meetings, quality improvement initiatives, and training sessions * Support clinic operations by contributing to a positive, team-oriented environment REQUIRED QUALIFICATIONS * Active Oregon license as a Nurse Practitioner*, PA-C, MD, or DO ( or eligibility to obtain ) * Board certification in Family Medicine, Internal Medicine, or applicable specialty * Strong interest in preventive, functional, and lifestyle medicine * Excellent communication and patient relationship skills * Comfortable working in a fast-paced, collaborative environment * Proficient in using electronic health records *Nurse Practitioner Licenses Accepted: * FNP- Family Nurse Practitioner * AGNP- Adult Gerontological Nurse Practitioner * AGNP- PC- Adult Gerontological Nurse Practitioner Benefits: * Medical, Dental, Vision, and Life Insurance * 401(k) Retirement Plan with Company Match * Voluntary LTD, FSA, Accident, Critical Illness * Paid Time Off and Paid Holidays * Employee and Family Discounts * Paid Parental Leave
