Registered Nurse (RN)- Weekend

About the Position Registered Nurse (RN) – Home Care Schedule & Territory Schedule: Saturday, Sunday and 1 weekday 8:00am – 5:00pm. Territory: Dublin, Lewis Center, Plain City, Upper Arlington, Polaris, Alexandria, Blacklick, New Albany, Newark, Sunbury, Utica, Westerville, Hillard, Grove City, West Jefferson, OSU, Grandview Hts. Responsibilities Plan of Care: Observe and monitor patient conditions; perform OASIS assessments; develop and adjust individualized care plans. Minimize Patient Risk: Administer medication as prescribed; prioritize visits for high‑risk patients to reduce re‑hospitalizations. Continuity of Care: Manage multidisciplinary care, ensure seamless admissions, transfers, and discharges. Family Educator/Advocate: Educate patients and families on disease processes using teach‑back methods. Policies & Documentation: Complete all clinical documentation per agency protocol and Medicare/Federal guidelines; collaborate with the interdisciplinary team and physicians. Rules & Regulations: Follow agency policies, procedures, rules and regulations; communicate schedule changes with schedulers or supervisors. Operations: Attend in‑service training and mandatory agency meetings. Hard & Soft Skills Compassionate communicator with a positive attitude. Patient, observant, and detail oriented. Can work autonomously and manage time efficiently. Requirements Registered Nurse with current license in the state of employment. Minimum one year of RN experience in an acute care setting. Home‑care experience preferred. Valid driver’s license and auto insurance named in your name as a driver. Capable of meeting all physical demands of the role.
