Registered Nurse - Specialty Clinic (Vascular)

This Registered Nurse - Specialty Clinic (Vascular) position is located in the Specialty Service at the Augusta, ME location. This position is full-time at 40 hours per week. Duties include but are not limited to:

• Assess, plan, and implement individualized care for patients with vascular disorders, such as peripheral artery disease, aneurysms (both high-risk and chronically stable), carotid stenosis, Hemodialysis access, and other related conditions.
• Monitor and care for post-operative vascular surgery patients, providing wound care and vigilant observation for potential complications.
• Ensure timely and appropriate follow-up's and imaging are ordered, support effective resource utilization and management, and identify the need for additional prosthetic equipment as part of the comprehensive care process.
• Supports cohesive patient flow throughout the clinic by collaborating closely with the vascular surgeon, vascular sonographer, and vascular MSAs.
• Assess each Veteran's disease severity, socioeconomic status, and available resources to deliver patient-centered care that is coordinated and tailored to individual needs.
• Educate patients and families about vascular diseases, prevention, post-surgical care, and lifestyle modifications.
• Coordinate and document patient care, utilizing VA electronic health records systems in compliance with VA and HIPAA standards.
• Obtain medical records and imaging from external healthcare facilities, including those located out of state, and ensure timely scanning and integration of these documents into the VA electronic health record system.
• Serve as an active member of the interdisciplinary team by collaborating with vascular surgeons, physicians, wound care specialists, the primary care team, and other specialists and healthcare professionals to deliver cohesive, patient-centered care.
• Facilitate care escalation and advocacy across the entire care team, within the VA and through community care partners, by implementing clear communication and evidence-based risk stratification.
• Comprehensive tracking and care coordination for patients and those receiving surgical care in the community, along with data collection and management for all vascular surgery cases.
• Participate in care planning, quality improvement initiatives, and professional development.
• Adhere to infection control standards safety protocols, and hospital policy.

Work Schedule: Full-Time, Monday - Friday, 7:30am - 4:00pm
