Social Worker (Program Coordinator)

Licensed Clinical Social WorkerDuties and responsibilities include but are not limited to: Program ManagementProvides administrative oversight of the M2VA Program to include planning, establishing, and implementing policies and procedures; developing program goals and objectives; and monitoring, operating, and evaluating the functioning of the program.Follows Veterans Health Administration (VHA) policy requirements and develops local systems to identify and track post 9/11 Servicemembers/Veterans (SM/Vs) served by the facility and community who belong in this special priority group.Coordinates comprehensive program elements and services for preventive mental and physical health evaluations for program participants.Ensures that the M2VA Program is interdisciplinary and integrated with the inpatient and outpatient medical programs.Establishes collaborative relationships with physicians, psychologists, nurses, social workers, and other disciplines to foster their involvement in the care of Veterans in the M2VA Program.Manages the daily operation of the facility M2VA Program and provides accountability for program effectiveness and modification of service patterns to enhance customer satisfactionOversight Of Case Management ServicesThe M2VA Program Manager is responsible for and may supervise M2VA nurse and social worker case managers and facility Transition Patient Advocates.Ensures that all Post 9/11 SMNs are screened for the need for case management services and that those in need of nurse and/or social worker case managers are assigned accordingly particularly those who are severely injured or ill.Coordinates the assembly of the Inter-Disciplinary Care Management Review Team (IDCMRT) Meeting.Manages, coordinates, and advocates for the provision of individual and family treatment as well as other services as needed.Patient, Stakeholders, and Staff EducationProvides consultation to other staff on the special needs of Post 9/11 SM/Vs.Provides education to Post 9/11 SMNs on VHA health care services available at the facility and about services available within the VISN and in the local community.Work Schedule: Monday Through Friday, 8:00 a.m. -4:30 p.m.Recruitment/Relocation Incentives (Sign-on Bonus): Not AuthorizedPermanent Change of Station (Relocation Assistance): Not AuthorizedPay: Competitive salary and regular salary increasesPaid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VAInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)Telework: Available, Ad-Hoc as determined by the agency policy.Virtual: This is not a virtual position.Functional Statement #: 93729-0Permanent Change of Station (PCS): Not Authorized
