Work Environment

This position requires you to work onsite at our rehabilitation facility in Victorville, CA, providing hands-on care and support to patients. You will be surrounded by a dedicated team of healthcare professionals in a collaborative and patient-centered setting. Working Monday to Friday from 2 PM to 8 PM, you will experience a balanced work schedule that supports your professional growth while allowing for personal health and well-being.

Key Responsibilities

• Deliver personalized, patient-centered physical therapy by managing the entire therapeutic process for an assigned group of patients in close coordination with the healthcare team.
• Conduct thorough patient assessments to identify functional limitations, develop, and implement individualized care plans aimed at achieving specific rehabilitation goals.
• Provide direct therapeutic interventions, ensuring that patients receive the appropriate level of care throughout their rehabilitation journey.
• Perform evaluations and treatment strategies that align with established best practices and ethical standards while adhering to legal guidelines.
• Engage in patient education, instructing patients and their families/caregivers on home exercises, use of therapeutic devices, and strategies for self-management.
• Prioritize caseloads and coordinate daily staffing needs to optimize patient care and therapy outcomes.
• Document patient progress and outcomes to provide relevant information for re-evaluation and ongoing treatment adjustments.

Patient Population / Client Focus

You will be instrumental in developing tailored approaches to care, considering each patient's unique situation, progress, and goals. Qualifications

• A Bachelor's Degree in Physical Therapy or equivalent.
• Current state licensing as a Physical Therapist in California.
• Basic Cardiac Life Support (BCLS) certification must be obtained within the first 30 days of employment.
• Strong communication and interpersonal skills to effectively liaise with patients and team members.

Preferred Experience

• Clinical experience in a rehabilitation facility or similar healthcare setting is preferable but not required.
• Familiarity with advanced therapeutic modalities and best practices in patient care.
• Experience in working with diverse patient populations is a plus.
