Responsibilities
- Administer prescribed medications and document administration in the resident’s medical record following policy and procedure.
- Monitor residents for changes in condition, report findings to the supervising nurse, and implement care plans.
- Provide routine and emergent bedside nursing care, including wound care, vital sign checks, and symptom management.
- Assist residents with activities of daily living and support rehabilitation or restorative therapies as directed.
- Document care accurately, maintain charting integrity, and ensure infection control and safety standards are followed.
- Collaborate with a multidisciplinary team to support discharge planning, care transitions, and resident well‑being.

Requirements
- Current Licensed Practical Nurse license in New York (active).
- Minimum experience in a long-term care or skilled nursing setting is preferred.
- Ability to perform standard nursing procedures and use an electronic health record system.
- Excellent communication skills and the ability to work within a team in a fast-paced care environment.
- Availability to work a 7:00 a.m. to 3:30 p.m. shift and meet assignment schedule.
- Diploma or associate degree in nursing (LPN).
