Responsibilities
- Administer prescribed medications and treatments following physician orders and facility policy
- Monitor residents’ conditions, document findings, and report changes to the nurse supervisor promptly
- Perform wound care, dressing changes, and basic nursing tasks as needed
- Assist with activities of daily living (ADLs) and provide resident-centered care
- Observe and document vital signs, intake/output, and maintain accurate charting
- Collaborate with the interdisciplinary team to ensure safety, infection control, and quality of care

Requirements
- Active Licensed Practical Nurse license in New York
- Current Basic Life Support (BLS) certification
- Minimum 1 year of licensed practical nursing experience in a long-term care setting
- Graduation from an approved LPN program (diploma or degree)
- Ability to work nights and commit to guaranteed hours
- Strong communication and documentation skills; ability to work with a multidisciplinary team
