Responsibilities

- Administer prescribed medications and monitor responses

- Assess resident status, document care, and report changes to the supervising clinician
- Communicate effectively with the interdisciplinary team to ensure continuity of care and safety

Requirements
- Active New York LPN license
- BLS certification (American Heart Association)
- Minimum 2 years of experience in a long-term care setting
- Proficiency in medication administration and direct patient care
- Ability to work independently, think critically, and collaborate with CNA staff and the care team
- Ability to work 12-hour shifts in a travel assignment and adapt to a long-term care environment
