Position Overview: As a Full-Time Case Manager, you will play a crucial role in our outpatient center, serving as a liaison between clients, treatment providers, and community resources to ensure comprehensive and coordinated care. You will work closely with clients to develop and implement individualized care plans, discharg eplanning, provide support and advocacy, and connect them with resources to address their needs.

Responsibilities:

• Conduct comprehensive assessments to identify clients' needs, strengths, and barriers to recovery.
• Develop individualized care plans in collaboration with clients and the treatment team.
• Coordinate services and referrals to internal and external resources, including housing, employment, healthcare, and social services.
• Provide ongoing support and advocacy to clients, assisting them in accessing and navigating available resources.
• Monitor client progress and adjust care plans as needed to ensure continuity of care and positive outcomes.
• Maintain accurate and timely documentation of client interactions, assessments, and service referrals.

Qualifications:

• Bachelor's degree in Social Work, Psychology, Counseling, or related field preferred;
• Previous experience in case management, social work, or related field preferred.
• Strong understanding of addiction treatment principles and best practices.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work effectively in a multidisciplinary team environment.
• Compassionate and empathetic approach to working with clients.
